Due to medical illness, Personal Family Medicine has been officially closed July 21st, 2008. Your records are being held at Ascension Global and you can request your records by sending an email to helpdesk@ascensionglobal.com or by calling (480) 213-6147. You will be asked to fill out a HIPPA form which needs to be notarized, in order to protect your confidentiality as best possible. There will be a minimal charge as the data center has to keep these records for many years.

On a personal note, I want to wish everyone health and happiness in the years to come. Although I will be returning home to Michigan, I will remember each of you who visited Personal Family Medicine with great fondness. I was very fortunate to have such wonderful patients, and learned a great deal from many of you. I hope to be well enough to return to work in a year or so, and pass to others some of the lessons you taught me.
